Job Description

RN Case Manager

Provide case management services to a specified panel of patients and as requested.
Through chart review, physician and caregiver interviews, manage patients across the continuum.
Apply appropriate payer based criteria to patients.
Manage resource consumption.
Work with Social Worker/Discharge Planner as part of a team and take responsibility as Team Leader.
Monitor and assess patient outcomes, coordinate clinical care and evaluate successfulness of discharge plan.

Requirements:
AA Degree, WA State RN License, Two years recent hospital experience.
